The Envelope 4 Life program is another method to provide emergency response crews with best possible patient infomation. It's a universally recognized by all fire departments and emergency care crews.
Follow the directions below to create your at-home kit for the Envelope 4 Life program, or pick up a kit at one of our public events in your area.
| 1) Download and print the attached form and fill out the medical information requested for each resident of your home. |

| 2) Put the information in an envelope and write: "Envelope for Life" on it and place in a plastic zip bag. |
 |
| 3) Wipe dry the inside rightward wall of your refrigerator and tape the form in a plastic zip bag to the side. |
 |
| 4) Download and print the attached small logo. Cut the logo as indicated, and then tape it to the outside top left corner of your refrigerator's door to alert emergency medical personnel that the envelope can be found inside. |

| 5) Be sure to update the form regularly. |
